一名无HIV感染的80岁男性患者主诉发热、进行性呼吸困难及咳嗽咳痰5周。尽管经过了抗生素治疗（羟氨苄青霉素/克拉维酸与阿奇霉素联用，后改为左氧氟沙星，哌拉西林/他唑巴坦与阿米卡星联用，然后改用替加环素），但是，相隔25天的CT扫描仍显示双肺实变进展（图A和B）。肺泡支气管灌洗液的微生物学检查结果（需氧培养，分支杆菌培养，Ziehl-Neelsen染色）均为阴性。因不明原因肺泡炎经胸腔镜行左下肺活检。标本显示粘液纤维组织形成的结节充满肺泡腔及细支气管（Masson小体，图C中箭头所示），间质纤维化，肺实质中局灶性分布泡沫性巨噬细胞，符合机化性肺炎的表现。对切除的肺标本进行Ziehl-Neelsen染色可见抗酸杆菌（图D中箭头所示），培养分离到耐异烟肼的结核分枝杆菌。这一不寻常的病例表明肺结核可以表现为机化性肺炎。
